Aging Life Care/Geriatric Care Manager Position for RN

Do you want to work as a consultant to solve the growing challenges facing older adults and their families with dementia and find real solutions?

Elder at Home, LLC, located in Pawtucket, RI, is expanding and seeking skilled clinicians to join our practice. Elder at Home works collaboratively with families on challenging situations, family dynamics, current and future planning, while offering a fulfilling and flexible work schedule.

Care managers at Elder at Home provide client-centered care management services to older adults and their families, including comprehensive in-home assessments, development and oversight of detailed care plans, consultation to families, advocacy for older adults in institutional (nursing home, assisted living communities) and in-home settings.

Ideal candidates have excellent clinical skills and are well versed or willing to learn how to advocate with insurances, including Medicare, Medicare Advantage Plans and Medicaid. Individuals will be trained and become proficient with opening up long-term care policies and assisting with Medicaid applications.

Individuals must have strong writing skills to complete comprehensive reports, be knowledgeable of community resources and be willing to research and learn what they need to know. Initiative, independence and a “can do” attitude a must.

Elder at Home care managers learn to work and think “out of the box.” Weekly supervision will be provided. The case manager role requires individuals who can work independently and serve as a compassionate advocate, while maintaining collegial relationships with medical and community providers.

Case managers begin part-time with opportunity for growth; as caseloads increase, hours may be increased, if desired.

Elder at Home LLC serves all of Rhode Island and neighboring MA towns and cities.

Qualifications:

  • AD, BS, or MS in Nursing.
  • Preferred 5-7 years of strong experience working with older adults in multiple settings.
  • Professional license supported by meeting the state requirements for annual CEU education.
  • Knowledge of community resources.
  • Vehicle with valid license and insurance. Mileage reimbursement.
